Please assist us by providing the following useful information at the time of your first visit:

• Any referral from your dentist including X-rays taken within the past six months.
• Insurance card and\or specific information required to verify orthodontic benefits. You may also provide this information at the time of scheduling your consultation appointment. This will allow us to have all benefits verified prior to your arrival. By providing this information, we will be able to give you an estimate of your out of pocket costs. In some instances, you may be fully covered, especially if there is dual insurance coverage.
